Role and Facility Information

This is an exciting opportunity to take over a partial patient panel from a highly regarded family physician transitioning to retirement within the next year. You'll have the chance to work alongside them during the transition, ensuring continuity of care while building out your own panel.

As a Family Physician here, you will:

- Provide full-scope family medicine, from pediatrics to geriatrics.
- Engage in shared decision-making with patients and care team to promote holistic, patient-centered care.
- Collaborate with our interprofessional team to deliver integrated team-based primary care.
- Participate in community outreach and health promotion initiatives.
- Participate in shared after-hours on-call rotation every 9 to 10 weeks.
- Contribute to broader clinical activities, including quality improvement, clinical meetings, program development and other non-client specific activities requiring physician expertise.

Highlights of the role include:

- A seamless transition into an existing panel – Inherit 250-500 patients from a highly regarded retiring physician, with a gradual handover to ensure continuity of care.
- The flexibility to shape your practice – Build the remainder of your panel based on your clinical interests and expertise.
- A well-supported, efficient practice – Focus on patient care in a streamlined, team-based environment with strong administrative and clinical support.
- A modern, well-equipped space – Work in a fully equipped clinic featuring eight exam rooms, a collaborative clinician workspace, dedicated virtual care offices, and an on-site pharmacy.

This is more than just a healthcare clinic—we're a vibrant, community-governed, non-profit organization committed to delivering high-quality, team-based primary care. We serve a diverse patient population and provide a wide range of integrated services, including:

- Full-scope family medicine
- On-site dental services
- Pharmacy support
- A parent support program for families with children (ages 0-3)
- Our primary care team includes seven Family Physicians, two Nurse Practitioners, two Registered Nurses, five Medical Office Assistants, and on-site clinical pharmacists, all working together to provide comprehensive patient-centered care.

Why Join?

We offer an exceptional practice environment where you can focus on patient care while maintaining a sustainable work-life balance. Here's what sets us apart:

- A Supportive, Team-Based Culture – Work alongside experienced clinicians and a skilled interprofessional team in a collaborative and inclusive setting.

- Diverse & Engaging Patient Population – Engage with a broad range of patients across all age groups and health conditions, including youth, mental health, addictions, refugees, and frail elderly patients.

- Work-Life Balance – Reasonable patient loads and a supportive administrative team to reduce paperwork burdens.

- Competitive Compensation – Physicians are currently on the Longitudinal Family Practice Payment Model. Split is competitive and negotiable based on working hours. Inquire about signing bonus for long-term commitments.

- User-Friendly Technology & Information Systems – We use Oscar OSP with Cortico online booking to streamline administrative tasks. Our clinic supports the use of AI scribe technology and other tools to optimize workflow.

- Innovation & Quality Improvement – We encourage new ideas to improve patient care and team workflows—and we'll support you in bringing your initiatives to life.
