Operating within the Hub and Spoke model co-led by Fred Victor and the Downtown East Toronto Ontario Health Team (DET OHT), the Manager is accountable for RPCHC’s HART Hub service delivery, staff management, operational planning, quality, and compliance with organizational and funder requirements.

The Manager leads an interdisciplinary team of Community Health Workers, Outreach Workers, Primary Care, and Social Work and works collaboratively with the HART Hub Project Coordinator, Spoke Partners and internal leadership to support integrated, low-barrier, trauma-informed mental health and addiction treatment and recovery services. The position maintains clear RPCHC operational accountabilities within the broader partnership model.

Responsibilities Include:

  • Provide management oversight of RPCHC’s HART Hub operations, ensuring alignment with program objectives, funding requirements, organizational policies and service standards.
  • Lead the coordination of an interdisciplinary team of Community Health Workers, Outreach Workers, Primary Care providers and Social Workers to deliver integrated, client-centred care.
  • Establish operational priorities, staffing coverage, workflows and service coordination processes to respond to changing client and community needs.

People Leadership & Staff Management

  • Provide direct supervision and leadership to RPCHC HART Hub staff.
  • Lead recruitment, onboarding, scheduling, workload assignment, regular supervision, performance evaluations, coaching and staff development in accordance with organizational policies and applicable collective agreement requirements.

Program Planning, Quality & Evaluation

  • Lead operational planning, implementation, monitoring and continuous improvement for RPCHC’s HART Hub services.
  • Ensure accurate and timely collection, documentation and reporting of client and program data using designated systems.
  • Monitor program indicators, service trends and outcomes; identify gaps and implement improvements based on evidence, client needs and partner feedback.

Financial, Funder & Resource Management

  • Monitor program resources, expenditures and funding deliverables in collaboration with the Director and Finance, within approved budgets and delegated authority.
  • Ensure accurate processing and review of program-related financial documentation and support funding compliance and reporting.

  • Represent RPCHC in operational partner meetings and contribute to cross-agency service integration while respecting each partner’s mandate and accountability.
  • Collaborate with the HART Hub Project Coordinator on implementation initiatives, partnership engagement and reporting; clarify operational roles and escalate governance or strategic issues appropriately.

Required Knowledge, Skills, and Abilities:

  • Bachelor’s degree in Social Work, Nursing, Social Sciences, Community Development, Public Health or a related field, or an equivalent combination of education and experience.
  • At least three (3) years of relevant experience in community health, harm reduction, mental health or social services, including program coordination or frontline supervision; demonstrated management-level accountability is preferred.
  • Experience supervising multidisciplinary frontline teams, including scheduling, coaching, performance management and resolution of complex operational issues.
  • Experience serving people experiencing homelessness, substance use, mental health concerns or other complex social needs to access treatment and pursue individualized recovery goals.
  • Experience with program monitoring, data reporting, partnership coordination and service improvement.
  • Experience coordinating care with community partners to strengthen treatment pathways, support continuity of care and advance client recovery.
  • Strong understanding of mental health and substance use treatment and recovery approaches, harm reduction, trauma-informed care, anti-oppression, anti-racism and culturally safe practice.
  • Demonstrated leadership, communication, problem-solving, conflict resolution and sound judgment.
