What Primary Health Care Entails
The Primary Health Care Program provides comprehensive, team-based primary care services to a diverse patient population. Working within the Community Health Centre model, providers collaborate across disciplines to deliver integrated, person-centred care that addresses both clinical needs and the social determinants of health. Services emphasize health promotion, disease prevention, continuity of care, health equity, and improved access for individuals who experience barriers to care.
What You’ll Do

The NP delivers culturally responsive care both independently and in collaboration with an interprofessional team, with a strong commitment to culturally safe, equitable, and client-centred care. This position requires availability to provide services one evening per week until 8:00 p.m.
The NP is responsible for delivering primary health care services in accordance with the Standards of Practice for Registered Nurses in the Extended Class and the Expanded Nursing Services for Patients Act. Providing clinical services requires the NP to manage a diverse range of health conditions while addressing the complex needs of the populations served.
The NP plays an active role in the development, implementation, monitoring, and evaluation of programs and services, ensuring accessible and high-quality care for individuals with diverse health, social, cultural, and economic needs.
