SE Health is Hiring a Registered Practical Nurse (RPN) - Journey Home Hospice

Great benefits and a team that supports your goals

Windsor shelters are often unable to provide planned palliative care deaths in their facilities and homeless individuals are likely to lack the family and social support networks that assist in providing care in the community. As a result, these individuals are dying on the street and in hospitals. Through the partnership with Assisted Living Southwestern Ontario (ALSO) and the City of Windsor, we now have a 4 bed cluster of residential hospice beds.Windsor satellite site provides accessible, equitable end-of-life care for people facing barriers to traditional services, including those experiencing homelessness or marginalization. Care is evidence-based, harm-reduction focused, trauma-informed, and culturally responsive, honouring each person’s dignity, lived experience, values, and needs.
