Position Summary

The Housing First / HOME Case Manager supports Indigenous and other youth experiencing homelessness or housing instability to obtain and maintain safe, affordable housing while building stability, belonging, and independence.

Housing First is the primary housing entry point, while HOME provides a less-intensive extension of care as youth move toward greater stability and adulthood. Support is not conditional on engagement.

The Case Manager provides person-centred, youth-led, culturally grounded, and strengths-based case management, while developing supportive relationships with participants, landlords, community partners, Elders, Knowledge Keepers, and other service providers.

The position also provides leadership, mentorship, and support to the Housing First staff team and contributes to the effective operation and ongoing development of the program.

Key Responsibilities and Accountabilities
The Housing First / HOME Case Manager's responsibilities include, but are not limited to:

· Provide in-home support to caregivers and families participating in the Home Fire program.

· Conduct assessments of home environments and identify areas that may benefit from improved organization, structure, cleanliness, or functionality.

· Collaborate with families to develop practical and achievable plans to improve the flow, safety, and function of the household.

· Support caregivers in creating and maintaining household organization systems, including storage solutions, routines, schedules, and family responsibilities.

· Assist families in establishing household processes and routines that support daily functioning and reduce family stress.

· Develop meal plans and support families in creating sustainable weekly meal preparation routines.

· Provide coaching related to household management, budgeting for household needs, and food planning when appropriate.

· Assist with light housekeeping tasks alongside families to model and support the development of healthy household practices.

· Coordinate the removal, disposal, donation, or relocation of unnecessary household items, boxes, furniture, or clutter as appropriate.

· Identify environmental factors within the home that may impact family wellness and work collaboratively with families to address them.

· Support families in accessing community resources and services that can assist with improving household stability and family well-being.

· Develop trusting and supportive relationships with caregivers and family members.

· Work collaboratively with Home Fire team members to implement family support plans and contribute to identified goals and outcomes.

· Provide timely and accurate documentation and reporting, including activities, interventions, goals, and outcomes.

· Participate in team meetings, case discussions, and other program-related activities.

· Adhere to established documentation, confidentiality, and program standards.

· Participate in training, professional development opportunities, and compliance requirements as defined by organizational policy.

· Perform other duties as assigned.

Qualifications / Experience

· Lived/living experience is strongly valued, including experience with homelessness, housing instability, poverty, child welfare, or other systems and experiences that impact youth.

· Strong understanding and appreciation of Indigenous histories, cultures, identities, and ways of knowing, including the impacts of colonization and the Truth and Reconciliation Commission of Canada’s Calls to Action.

· A strong connection to Indigenous community, culture, identity, and ways of knowing is valued.

· Knowledge and experience with Housing First, harm reduction, trauma-informed, strengths-based, solution-focused, and youth-centred approaches.

· Demonstrated ability to build positive relationships, community connections, networks, and partnerships.

· Strong interpersonal, communication, advocacy, organizational, and relationship-building skills.

· Demonstrated leadership skills and the ability to work collaboratively with youth, staff, community partners, Elders, Knowledge Keepers, and other service providers.

· Ability to support youth choice, self-determination, independence, and cultural connection.

· Knowledge of housing, tenancy, and community resources, with the ability to connect youth to appropriate supports and services.

· Strong crisis intervention, de-escalation, harm reduction, and conflict resolution skills.

· Ability to work a flexible schedule, including days, evenings, and weekends, based on program and participant needs.

· Demonstrated proficiency with Microsoft Office at an intermediate level. Experience with HIFIS is an asset.

· Valid driver's licence and access to a reliable vehicle with required insurance coverage.

· Satisfactory Criminal Record Check with Vulnerable Sector Search, Child Abuse Registry check with acceptable Prior Contact checks, and Adult Abuse Registry check, completed within the required timeframe.

· Applicable certifications are considered an asset, including First Aid/CPR Level C, ASIST, Mental Health First Aid, SafeTALK, and De-escalation Training.

Education (Degree/Diploma/Certificate)

· Three years of experience working with youth, preferably youth experiencing homelessness, housing instability, poverty, child welfare involvement, or other barriers to stability and independence.

· Post-secondary education in Social Work, Human Services, Education, Indigenous Studies, or a related field is preferred. Equivalent education, training, and experience will be considered.

· An equivalent combination of education, relevant employment experience, and/or lived experience will be considered.

· Emergency First Aid and CPR Level C certification required.

· ASIST (Applied Suicide Intervention Skills Training), Crisis De-escalation Training, or willingness to obtain required certifications is an asset.
